Responsibilities
- Perform comprehensive remote patient assessments and triage via secure telehealth platforms.
- Administer medication and treatment plans according to physician orders.
- Document patient interactions, assessments, and care plans accurately and timely in EMR systems.
- Collaborate effectively with physicians, nurse practitioners, and multidisciplinary care teams.
- Provide patient education and support for chronic disease management and preventative care.
- Respond promptly to patient inquiries and escalate critical issues immediately.
Qualifications
- Active Registered Nurse (RN) license in Illinois (or a compact state).
- Minimum 2 years of clinical experience in a relevant specialty (e.g., Med-Surg, ICU, or Telehealth).
- Strong proficiency with electronic health records (EHR) and telehealth software (e.g., Epic, Cerner).
- Excellent verbal and written communication skills.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Ability to work independently in a remote environment with high levels of self-motivation.
