Responsibilities
- Conduct comprehensive virtual patient assessments and triage calls to determine appropriate levels of care.
- Provide accurate health education and medication management instructions to patients via secure video or phone.
- Document all patient interactions, assessments, and care plans in the Electronic Health Record (EHR) system in real-time.
- Collaborate effectively with physicians, nurse practitioners, and case managers to ensure continuity of care.
- Identify potential health risks and escalations, initiating appropriate referrals or emergency protocols when necessary.
- Maintain strict compliance with HIPAA regulations and company telehealth protocols at all times.
Qualifications
- Current and active Registered Nurse (RN) license in the State of Indiana.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Minimum of 2 years of nursing experience, preferably in a clinical, telehealth, or case management setting.
- Strong computer proficiency and comfortable navigating electronic medical records systems.
- Excellent verbal communication skills with the ability to build rapport with patients remotely.
- Self-motivated individual capable of working independently in a home office environment.
