Responsibilities
- Conduct comprehensive patient assessments via secure telehealth platforms.
- Develop and implement individualized care plans in collaboration with physicians and specialists.
- Administer medications and treatments as prescribed, ensuring safety and accuracy.
- Maintain accurate and up-to-date electronic health records (EHR).
- Provide patient education and counseling on chronic disease management and wellness.
- Respond to urgent patient inquiries and coordinate referrals to in-person services when necessary.
Qualifications
- Valid Registered Nurse (RN) license in the State of New York.
- Minimum of 2 years of clinical experience in a medical or telehealth setting.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong proficiency with electronic health records (EHR) systems.
- Excellent communication skills and ability to build rapport remotely.
- Compassionate demeanor with a focus on patient-centered care.
