Responsibilities
- Conduct Comprehensive Patient Assessments: Evaluate patient conditions remotely and determine appropriate levels of care or referrals.
- Manage Patient Care Plans: Develop, implement, and update individualized care plans in collaboration with the care team and patients.
- Administer Medications and Treatments: Accurately prescribe and verify medications according to physician orders and hospital protocols.
- Document Clinical Records: Maintain precise and timely electronic health records (EHR) detailing patient interactions and outcomes.
- Communicate with Healthcare Team: Liaise effectively with physicians, nurses, and other staff to ensure seamless patient transitions and continuity of care.
- Provide Patient Education: Educate patients and families on disease management, medication side effects, and preventative health strategies.
- Respond to Triage Requests: Manage incoming remote patient queries and urgent care needs with professionalism and urgency.
Qualifications
- Active RN License: Valid Registered Nurse license in the District of Columbia (or compact state eligible).
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required.
- Experience: Minimum of 2 years of nursing experience in a hospital or clinical setting preferred.
- Technical Proficiency: Must be comfortable navigating Electronic Health Records (EHR) systems and remote communication tools (e.g., Zoom, Microsoft Teams).
- Communication Skills: Excellent verbal and written communication skills with a patient-centered approach.
- Time Management: Ability to prioritize tasks effectively in a fast-paced, remote environment.
- Reliability: High level of professional accountability and adherence to HIPAA regulations.
