Responsibilities
- Conduct virtual patient assessments and develop individualized care plans
- Manage telehealth consultations for acute and chronic conditions
- Coordinate care transitions and collaborate with community providers
- Document clinical encounters in EMR systems with 100% accuracy
- Participate in on-call rotation for urgent patient concerns
- Maintain compliance with HIPAA and telehealth regulations
- Contribute to quality improvement initiatives
Qualifications
- Active Ohio RN license (Compact License preferred)
- 2+ years clinical nursing experience
- BLS/CPR certification
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- Strong critical thinking and decision-making skills
- Excellent written/verbal communication abilities
- Experience with EMR systems (Epic, Cerner)
- Ability to work independently in a remote setting
