Responsibilities
- Provide remote patient assessment and triage via telehealth platform
- Coordinate care plans with in-person clinical teams
- Administer virtual medication management & education
- Document patient encounters in EMR systems
- Collaborate with multidisciplinary care teams
- Respond to patient inquiries within service level agreements
- Maintain HIPAA compliance in all virtual interactions
Qualifications
- Active Texas RN license (required)
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care certification preferred
- Strong assessment & critical thinking skills
- Experience with EMR systems (Epic a plus)
- Excellent communication & computer literacy
- BLS/CPR certification (current)
