Responsibilities
- Provide direct patient care, monitoring vital signs, and administering medications as prescribed by physicians.
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and mobility support.
- Document patient health information accurately, timely, and completely in electronic health records (EHR).
- Collaborate with physicians, therapists, and the multidisciplinary healthcare team to develop and implement patient care plans.
- Maintain a clean, safe, and sanitized environment to prevent infection and ensure patient comfort.
- Communicate effectively with patients, families, and caregivers regarding care status, treatment plans, and updates.
Qualifications
- Current, unencumbered RN or CNA license in the State of Oregon.
- CPR/BLS certification (ACLS preferred for Registered Nurses).
- Previous experience in a clinical, home health, or hospital setting is highly preferred.
- Strong computer proficiency and familiarity with Electronic Health Records (EHR) systems (e.g., Epic, Cerner).
- Excellent verbal and written communication skills with a focus on empathy and clarity.
- Ability to work well under pressure and demonstrate critical thinking in emergency situations.
