Responsibilities:

- Conduct comprehensive patient assessments and triage via virtual platforms to determine appropriate care plans.
- Document patient interactions and medical histories accurately and timely in Electronic Health Records (EHR).
- Collaborate with physicians and healthcare providers to develop, implement, and evaluate patient treatment plans.
- Provide patient education and counseling on disease prevention, medication management, and healthy lifestyle choices.
- Monitor patient progress and escalates critical issues to on-call providers immediately.
- Maintain strict adherence to HIPAA regulations and organizational privacy policies.
- Participate in continuous quality improvement initiatives to enhance patient outcomes.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the District of Columbia (DC).
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required (new grads welcome).
- Basic Life Support (BLS) certification through the American Heart Association.
- Strong computer proficiency with experience in navigating electronic health record systems.
- Excellent verbal and written communication skills for remote patient interaction.
- Ability to work independently with minimal supervision while demonstrating critical thinking.
