Responsibilities
- Provide virtual patient monitoring and health assessments via telehealth platform
- Assist with medication reminders and wellness checks through digital communication
- Collaborate with RNs and physicians to coordinate patient care plans
- Document patient interactions accurately in electronic health records (EHR)
- Respond to patient inquiries via secure messaging systems
- Participate in virtual care team meetings and training sessions
- Maintain strict HIPAA compliance in all digital interactions
Qualifications
- Active Pennsylvania CNA certification in good standing
- Minimum 1 year of clinical experience in healthcare setting
- Proficiency with telehealth technology and EHR systems
- Excellent written and verbal communication skills
- Ability to work independently with minimal supervision
- Strong problem-solving and critical thinking abilities
- Valid Pennsylvania driver's license and reliable transportation
- Successful background check and drug screening
