Responsibilities
- Conduct comprehensive patient assessments via telehealth platforms and triage medical inquiries with accuracy and empathy.
- Document patient interactions, medical histories, and treatment plans in Electronic Health Records (EHR) systems in real-time.
- Collaborate with physicians, nurses, and specialists to develop and coordinate patient care plans.
- Provide health education and resources to patients to promote wellness and disease prevention.
- Manage incoming phone and digital communication channels, ensuring timely responses to all inquiries.
- Monitor patient status and escalate urgent cases to on-site providers or emergency services when necessary.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Maryland.
- Minimum of 2 years of nursing experience, preferably in a clinical or telehealth setting.
- Strong computer skills and proficiency with Electronic Health Records (EHR) systems.
- Excellent verbal and written communication skills with a focus on patient-centered care.
- Ability to work independently while maintaining strict adherence to hospital protocols and HIPAA regulations.
- BSN or Associate Degree in Nursing (ADN) required.
