Responsibilities
- Conduct comprehensive telephone and digital symptom assessments to determine the urgency of patient needs.
- Apply evidence-based clinical guidelines to triage patients accurately and route them to the appropriate level of care (Emergency, Urgent Care, or Home Care).
- Document patient interactions and medical history accurately and securely within the electronic health record (EHR) system.
- Collaborate with physicians, nurse practitioners, and other healthcare professionals to ensure continuity of care.
- Provide patient education and health coaching to promote wellness and self-management.
- Respond promptly to incoming inquiries and escalate critical cases immediately.
Qualifications
- Current and active AZ RN License (or compact license eligible).
- Minimum of 2-3 years of acute care nursing experience (ICU, ER, or Med-Surg preferred).
- Previous experience in a triage or telehealth setting is highly desirable.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong communication skills with the ability to remain calm under pressure and de-escalate tense situations.
- Proficient in using computer systems and medical software applications.
