Responsibilities
- Conduct Comprehensive Assessments: Perform detailed telephone and in-person triage evaluations to determine the acuity of patient symptoms and the urgency of required care.
- Prioritize Care: Utilize established protocols to triage incoming calls and direct patients to the appropriate level of care, including Emergency Department, Urgent Care, or primary care.
- Patient Advocacy: Act as the patient's voice, ensuring they understand their condition and the next steps in their treatment plan.
- Documentation: Accurately document all triage interactions, patient history, and care recommendations in the electronic health record (EHR) system.
- Collaboration: Communicate effectively with physicians, nurses, and other healthcare professionals to facilitate timely patient management.
- Education: Provide patient education on self-care management and preventative health measures over the phone.
- Quality Assurance: Participate in quality improvement initiatives and adhere to all regulatory standards and hospital policies.
Qualifications
- Licensure: Current, unencumbered Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the State of Louisiana.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 2 years of nursing experience, preferably in a clinical setting, ED, or triage environment.
- Skills: Strong critical thinking and decision-making skills under pressure; excellent verbal and written communication abilities.
- Technology: Proficiency with electronic health records (EHR) and computer systems.
- Professionalism: Demonstrated ability to handle high-volume calls with empathy and professionalism.
