Responsibilities
- Perform comprehensive remote patient health assessments and virtual vitals monitoring.
- Update and maintain accurate Electronic Health Records (EHR) and digital patient documentation.
- Coordinate care schedules and facilitate clear communication between patients, families, and medical staff.
- Provide virtual health education, medication reminders, and emotional support to patients.
- Ensure strict adherence to HIPAA compliance and remote data security protocols.
- Collaborate with the clinical team to adjust care plans based on remote patient feedback.
Qualifications
- Active and unencumbered Certified Nursing Assistant (CNA) license in the United States.
- Proven experience in clinical documentation and patient care coordination.
- Proficiency in Microsoft Office Suite and Electronic Health Records software (Epic, Cerner, AthenaHealth).
- Reliable high-speed internet connection and a private, quiet workspace.
- Excellent verbal and written communication skills for virtual interactions.
- Strong computer proficiency and ability to work independently in a remote environment.
