Responsibilities
- Conduct virtual health assessments and patient monitoring via telehealth platforms
- Coordinate care plans between patients, families, and healthcare providers
- Provide patient education on chronic conditions and medication management
- Document accurate clinical notes in electronic health records (EHR)
- Assist with scheduling virtual appointments and care transitions

Qualifications
- Active CNA certification in good standing
