Responsibilities
- Deliver telehealth support and virtual patient assessments via secure video platforms
- Coordinate care plans with registered nurses and physicians remotely
- Provide medication reminders and health education through digital channels
- Document patient interactions accurately in electronic health records (EHR)
- Monitor patient vitals and symptoms through telehealth monitoring devices
- Facilitate virtual care conferences with patients and families
- Respond to patient inquiries within SLA timeframes
Qualifications
- Active CNA certification in California (required)
- Minimum 1 year experience in telehealth or remote healthcare
- Proficiency with telehealth platforms and EHR systems
- Strong written and verbal communication skills
- Ability to work independently with minimal supervision
- Reliable high-speed internet and dedicated home workspace
- CPR/BLS certification (current)
