Responsibilities
- Provide high-quality, compassionate care to patients in their homes, adhering to state regulations and company standards.
- Monitor patient health status, recording vital signs, and reporting any changes to the supervising nurse immediately.
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, and mobility support.
- Maintain accurate and timely documentation of patient care plans, progress notes, and incident reports.
- Communicate effectively with patients, families, and the healthcare team to ensure holistic care.
- Adhere to strict privacy policies and HIPAA guidelines at all times.
Qualifications
- Current, unencumbered CNA license in the State of Ohio.
- Current Basic Life Support (BLS) certification (AHA).
- Previous experience in home health or acute care preferred.
- Strong computer skills and ability to navigate electronic health record (EHR) systems.
- Excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation (if required for site visits).
