Responsibilities
- Provide telehealth patient support and health education remotely
- Monitor patient vitals and medication adherence via digital platforms
- Coordinate care plans with interdisciplinary healthcare teams
- Conduct virtual wellness checks and symptom assessments
- Maintain accurate electronic health records (EHR)
- Facilitate communication between patients, families, and providers

Qualifications
- Active CNA license in New York State
