Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platform
- Monitor patient vitals and medication adherence remotely
- Collaborate with RNs and physicians to update care plans
- Train patients/families on home care procedures via video calls
- Document patient interactions in EHR systems in real-time
- Respond to patient inquiries within 2-hour SLA
- Conduct virtual wellness checks for assigned caseload
Qualifications
- Active California CNA license in good standing
- 1+ years experience in home health or telehealth
- Proficient with telehealth software (e.g., Doxy, Teladoc)
- Strong written/verbal communication skills
- HIPAA compliance certification
- Reliable high-speed internet and quiet home office
- Ability to pass background check and drug screening
- Spanish bilingual preferred
