Responsibilities
- Conduct virtual patient assessments via telehealth platform
- Develop and implement personalized care plans
- Coordinate with healthcare providers for seamless care transitions
- Provide health education and medication reminders
- Document patient interactions in EHR systems
- Monitor patient progress and report changes to clinical team
- Respond to patient inquiries within SLA timeframe
Qualifications
- Active CNA certification in good standing
- Minimum 1 year clinical experience

- Strong communication and documentation skills

- Valid CPR certification
