Responsibilities
- Provide telehealth support to patients via secure video calls
- Monitor patient vitals and medication adherence remotely
- Coordinate care plans between patients, families, and medical teams
- Document patient interactions in HIPAA-compliant systems
- Conduct wellness check-ins via phone/video for post-op/discharge patients
- Educate patients on home care procedures and health management
- Collaborate with RNs and physicians for care adjustments
Qualifications
- Active Ohio CNA certification
- 1+ years clinical experience (home health/hospital preferred)
- Strong communication and tech proficiency (EMR systems, video conferencing)
- Ability to pass remote work background check and drug screen
- Compassionate patient care approach
- Valid Ohio driver's license and reliable transportation
- Availability for evening/weekend shifts (as needed)
