Responsibilities
- Provide direct, compassionate patient care in home health settings, including assisting with Activities of Daily Living (ADLs) and personal hygiene.
- Monitor and record vital signs, such as blood pressure, temperature, and pulse, and report any significant changes to the Registered Nurse (RN) supervisor.
- Prepare and administer medications as prescribed, ensuring accurate documentation in electronic health records (EHR) systems.
- Communicate effectively with patients, families, and interdisciplinary healthcare teams to coordinate care plans and address patient concerns.
- Assist with light housekeeping and meal preparation to ensure a safe and comfortable living environment for patients.
- Participate in continuing education and training to maintain licensure and improve care delivery standards.
Qualifications
- Active Certified Nursing Assistant (CNA) license in North Carolina.
- BLS (Basic Life Support) Certification required.
- Minimum of 1 year of experience in a clinical or home health setting preferred.
- Proficient in using computer systems and electronic health records (EHR) for documentation.
- Strong interpersonal and communication skills with a focus on patient empathy.
- Ability to work independently and manage time effectively in a remote or home-based role.
