Responsibilities
- Provide virtual patient assessments via telehealth platform
- Coordinate care plans with RNs and physicians
- Monitor patient vitals and medication adherence remotely
- Document clinical notes in EHR systems
- Conduct wellness checks via video calls
- Assist patients with scheduling and resource navigation
- Collaborate with interdisciplinary care teams
Qualifications
- Active CNA certification in Texas
- 1+ years clinical experience
- Reliable high-speed internet and quiet workspace
- Experience with telehealth platforms (Epic preferred)
- Strong written communication skills
- Ability to work independently with minimal supervision
- BLS/CPR certification
