Responsibilities
- Provide direct patient care and assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Monitor and record patient vital signs, symptoms, and progress accurately in electronic health records (EHR).
- Administer medication as prescribed by the Registered Nurse (RN) and document administration logs.
- Collaborate with the interdisciplinary team, including RNs and therapists, to develop and update patient care plans.
- Perform light housekeeping and meal preparation tasks to ensure a safe and clean living environment for patients.
- Communicate effectively with patients and families to provide education and emotional support.
- Respond promptly to patient needs and emergency situations following established protocols.
Qualifications
- Certified Nursing Assistant (CNA) license active and in good standing in the state of residence.
- Current Basic Life Support (BLS) and CPR certification.
- Minimum of 1 year of experience in home health, long-term care, or hospital setting preferred.
- Strong computer skills and ability to navigate electronic health record systems (EMR).
- Excellent communication skills, both written and verbal.
- Valid driver's license and reliable transportation (for occasional in-person visits if required by role specifics).
- Compassionate demeanor with a strong commitment to patient privacy and dignity.
