- Communication Hub: Liaise effectively between patients, physicians, and nursing staff to escalate critical issues immediately and coordinate necessary in-person visits.
- Health Monitoring & Assessment: Monitor patient data remotely, identify trends, and proactively address potential health risks before they escalate.
- Compliance & Safety: Adhere strictly to HIPAA regulations, company telehealth protocols, and safety guidelines to protect patient privacy.
Qualifications
- Active Certification: Valid, unencumbered CNA Certification (State Licensure) required.
- Technical Proficiency: Must have a reliable high-speed internet connection, a quiet workspace, and basic computer skills.
- Experience: Minimum of 1 year of nursing experience or relevant home health care experience preferred.
