Responsibilities
- Conduct remote patient assessments via telehealth platforms to monitor health status and identify concerns
- Provide virtual health education and coaching to patients and caregivers on chronic disease management
- Coordinate care plans with multidisciplinary teams including RNs and physicians
- Document patient interactions accurately in electronic health records (EHR) systems
- Triage incoming patient communications and escalate critical issues appropriately
- Assist with medication reconciliation and refill authorization processes
- Participate in virtual care coordination for high-risk patient populations
Qualifications
- Active CNA license in good standing with state nursing board
- Minimum 2 years of clinical experience in acute care or long-term setting
- Proficient in telehealth technology and EHR systems (e.g., Epic, Cerner)
- Exceptional communication skills with ability to build rapport virtually
- Strong critical thinking and problem-solving abilities
- Valid BLS/CPR certification
- Ability to pass comprehensive background check and drug screening
