Responsibilities
- Conduct virtual health assessments via video calls
- Coordinate care plans with RNs and physicians
- Provide telephonic medication reminders
- Document patient interactions in EHR systems
- Connect patients to community resources
- Monitor vital signs remotely via IoT devices
- Advocate for patient needs across care teams
Qualifications
- Active California CNA certification required
- 1+ years clinical experience preferred
- Proficient with telehealth platforms (Zoom, Doxy.me)
- Strong written/verbal communication skills
- Comfortable with EHR systems (Epic, Cerner)
- Ability to pass remote background check
- CPR/BLS certification maintained
- Reliable high-speed internet connection
