Responsibilities
- Provide virtual patient monitoring via telehealth platforms
- Coordinate care plans with RNs and interdisciplinary teams remotely
- Conduct in-home health assessments and vital sign checks
- Assist patients with ADLs (Activities of Daily Living) in home settings
- Maintain accurate electronic health records (EHRs) remotely
- Respond to patient inquiries via telehealth channels
- Collaborate with family members for care continuity
Qualifications
- Active Texas CNA certification required
- Minimum 1 year clinical experience
- Proficient in telehealth technology (Zoom, Epic, etc.)
- Valid CPR/BLS certification
- Strong communication and documentation skills
- Ability to travel within Fort Worth metro area
- Flexibility for hybrid work schedule
- Pass background check and drug screen
