Responsibilities
- Provide virtual patient monitoring and health assessments via telehealth platform
- Assist with medication reminders and health documentation remotely
- Coordinate care plans with RNs and physicians
- Conduct periodic in-home health evaluations (Fort Worth area)
- Document patient interactions accurately in EHR systems
- Respond to urgent patient inquiries within service hours
- Participate in virtual team huddles and training sessions
Qualifications
- Active Texas CNA certification required
- Minimum 1 year clinical experience in healthcare setting
- Proficient with telehealth technology and EHR systems
- Strong communication skills (verbal/written)
- Ability to travel locally for patient visits (5-10 hrs/week)
- BLS/CPR certification
- Pass background check and drug screening
