Responsibilities
- Provide personalized care plans including medication management, meal preparation, and mobility assistance
- Conduct virtual health monitoring and telehealth consultations via our secure platform
- Document care activities and client progress using our HIPAA-compliant EHR system
- Coordinate with healthcare providers and family members for holistic care delivery
- Transport clients to medical appointments and community activities
- Assist with personal care, hygiene, and light housekeeping duties
- Respond to emergency situations following established protocols
Qualifications
- Minimum 1 year professional caregiving experience or CNA certification
- Valid Georgia driver's license with clean driving record
- Reliable transportation and smartphone with internet access
- Proficiency in telehealth platforms and digital care documentation tools
- CPR/First Aid certification (or willingness to obtain)
