Responsibilities
- Provide personalized in-home care including meal preparation, medication reminders, and light housekeeping
- Assist clients with mobility, bathing, and personal hygiene tasks
- Monitor client health status and document observations using our digital care platform
- Coordinate telehealth appointments and virtual wellness checks with remote clinical team
- Transport clients to medical appointments and community activities
- Implement personalized care plans and collaborate with family members and healthcare providers
- Maintain accurate care records and incident reports through our mobile app
Qualifications
- Minimum 1 year professional caregiving experience or CNA/HHA certification
- Valid Colorado driver's license and reliable transportation
- Proficient with smartphones/tablets for telehealth documentation
- CPR/First Aid certification (or willingness to obtain)
- Ability to pass comprehensive background checks and drug screening
- Strong communication skills for remote client interactions
- Compassionate demeanor and patience working with diverse populations
- Flexible availability including evenings and weekends
