Responsibilities
- Deliver personalized virtual care plans including medication reminders, wellness checks, and emotional support
- Coordinate with healthcare providers and family members for seamless client care
- Monitor client vitals and health status through digital platforms
- Document care activities accurately in our HIPAA-compliant system
- Provide companionship via video calls and digital engagement tools
- Assist with technology setup for remote care equipment
- Respond to client inquiries with empathy and professionalism
Qualifications
- Minimum 1 year professional caregiving experience
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) preferred
- Proficient with telehealth platforms and digital care tools
- Valid Arizona driver's license and reliable transportation
- Exceptional communication and interpersonal skills
- Ability to pass background check and drug screening
