Responsibilities
- Provide compassionate in-home care including personal hygiene assistance, meal preparation, and medication reminders
- Monitor client health status and document care activities using our mobile app
- Coordinate with healthcare providers and family members via telehealth platforms
- Assist with light housekeeping, transportation to appointments, and errands
- Develop personalized care plans through remote collaboration with our clinical team
- Respond to emergency situations following established protocols
- Maintain accurate electronic health records remotely
Qualifications
- Minimum 1 year professional caregiving experience or CNA certification
- Valid Pennsylvania driver's license with clean driving record
- Reliable transportation for in-home visits
- Proficiency with telehealth apps and electronic documentation systems
- CPR/First Aid certification (or willingness to obtain)
- Excellent communication skills for remote client/family interactions
- Ability to manage flexible schedule with in-person and remote components
- Pass background check and drug screening
