Responsibilities
- Provide personalized in-home care (bathing, dressing, medication management)
- Monitor vital signs and health status; report changes to clinical team
- Prepare nutritious meals and assist with dietary needs
- Transport clients to appointments (mileage reimbursement provided)
- Maintain accurate digital health records via remote portal
- Coordinate care plans with telehealth specialists
- Offer companionship and emotional support
- Perform light housekeeping tasks
Qualifications
- State-certified Nursing Assistant (CNA) or Home Health Aide (HHA)
- 1+ years professional caregiving experience
- Current CPR/First Aid certification
- Reliable transportation with valid driver's license
- Proficient with digital health records and telehealth platforms
- Exceptional communication and empathy skills
- Ability to pass comprehensive background check
