Responsibilities
- Provide in-home companionship and emotional support to clients in Phoenix metro area
- Assist with daily living activities (bathing, dressing, medication reminders)
- Coordinate care plans remotely using our digital health platform
- Transport clients to appointments (mileage reimbursed)
- Monitor health conditions and report changes to care team
- Light housekeeping and meal preparation
- Document care progress via HIPAA-compliant mobile app
Qualifications
- Minimum 1 year caregiver experience (CNA certification preferred)
- Valid Arizona driver's license and insured vehicle
- Reliable smartphone with data plan for remote coordination
