Responsibilities
- Conduct comprehensive virtual health assessments and prioritize patient cases based on urgency
- Provide evidence-based clinical guidance via phone, video, and secure messaging
- Collaborate with providers to coordinate care for complex patient cases
- Maintain accurate electronic health records while adhering to HIPAA compliance
- Develop and execute patient education plans for chronic disease management
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Minnesota with 3+ years clinical experience
- BLS certification and current ACLS/PALS preferred
- Proven expertise in triage protocols and emergency assessment
- Exceptional communication skills with diverse patient populations
- Proficiency in EMR systems and telehealth platforms
- Ability to work independently in a remote environment
- Experience with chronic disease management a plus
