Responsibilities
- Provide personalized in-home care including bathing, dressing, and meal preparation
- Monitor client health status and report changes to medical team
- Assist with mobility transfers and light housekeeping duties
- Administer prescribed medications under professional supervision
- Maintain accurate care records using our digital health platform
- Coordinate telehealth appointments when required
Qualifications
- Minimum 1 year professional home health aide experience
- Active District of Home Care Aide certification
- CPR and First Aid certification required
- Valid driver's license with reliable transportation
- Proficient with telehealth technology and apps
- Excellent communication and empathy skills
- Ability to work independently with minimal supervision
