Responsibilities
- Provide personalized assistance with daily living activities including bathing, dressing, and mobility support
- Monitor vital signs and document patient observations in our digital health platform
- Prepare nutritious meals and assist with medication management under supervision
- Transport clients to medical appointments and social activities using company vehicles
- Coordinate telehealth sessions and facilitate remote communication between patients and care teams
- Perform light housekeeping tasks and maintain safe, hygienic home environments
- Collaborate with nurses and therapists to implement personalized care plans
Qualifications
- State-certified Home Health Aide or willingness to obtain certification within 30 days
- Minimum 1 year professional caregiving experience with references
- Current CPR/First Aid certification and valid California driver's license
- Proficient with digital health records and telehealth technologies
- Ability to pass comprehensive background check and TB screening
- Excellent communication skills and fluency in English/Spanish preferred
- Reliable transportation and flexible availability for client needs
