In this role, you will serve as the primary point of contact for patients, coordinating care plans and ensuring seamless transitions between healthcare settings. Responsibilities
- Conduct comprehensive patient assessments and case reviews via telehealth and video conferencing tools.
- Develop, implement, and monitor individualized care plans in collaboration with physicians and interdisciplinary teams.
- Coordinate home health services, medical equipment, and community resources to ensure continuity of care.
- Monitor patient progress and adjust care strategies to meet clinical outcomes.
- Maintain accurate, up-to-date documentation in Electronic Health Records (EHR) systems.
- Provide education and emotional support to patients and their families regarding their health conditions.
- Communicate effectively with insurance providers and external agencies regarding patient status and authorizations.
Qualifications
- Valid CNA, LPN, or RN license in the state of Pennsylvania (active).
- Minimum of 2 years of clinical experience in nursing, home health, or case management.
- Strong proficiency with Microsoft Office Suite and electronic health record systems.
- Excellent verbal and written communication skills.
- Ability to work independently in a remote environment with high levels of self-discipline.
- Reliable high-speed internet connection and a quiet workspace.
