Responsibilities
- Conduct virtual patient assessments and develop care plans
- Provide telehealth consultations and medication management
- Coordinate care between patients, families, and healthcare providers
- Document electronic health records (EHR) with precision
- Monitor patient progress and adjust interventions as needed
- Educate patients on chronic disease management
- Respond to urgent patient inquiries within 2-hour SLA
- Collaborate with interdisciplinary care teams
Qualifications
- Active Michigan RN license (compact license accepted)
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience preferred
- BLS/CPR certification required
- Proficient with EHR systems (Epic, Cerner, etc.)
- Strong critical thinking and assessment skills
- Excellent written/verbal communication abilities
- Ability to work independently with minimal supervision
