Responsibilities
- Deliver high-quality virtual patient assessments and care coordination via telehealth platforms
- Monitor and document patient health status using electronic health records (EHR)
- Collaborate with interdisciplinary teams to develop and implement care plans
- Provide patient education on health management and medication adherence
- Respond to urgent virtual consultations and triage appropriately
- Participate in continuous quality improvement initiatives for remote care delivery
- Adhere to HIPAA compliance and telehealth best practices
Qualifications
- Active RN license in Colorado (or compact state with quick endorsement)
- Associate or Bachelor of Science in Nursing (ADN/BSN) required
- 0-2 years of clinical experience (new graduates encouraged)
- BLS certification required (ACLS preferred)
- Strong computer literacy and comfort with telehealth technology
- Excellent communication skills for virtual patient interactions
