Responsibilities
- Conduct comprehensive virtual patient assessments via telehealth platform
- Develop and implement personalized care plans for remote patient populations
- Provide clinical oversight and medication management through virtual consultations
- Coordinate care transitions between in-person and virtual settings
- Document patient encounters accurately in our EMR system
- Collaborate with on-site care teams for complex case management
- Participate in continuous quality improvement initiatives
Qualifications
- Active California RN license (unrestricted)
- Bachelor of Science in Nursing (BSN) required
- Minimum 2 years of acute care or telehealth experience
- BLS and ACLS certifications
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- Strong critical thinking and decision-making skills
- Excellent communication abilities across virtual platforms
- Experience with Epic EMR systems preferred
