Responsibilities
- Deliver comprehensive virtual patient assessments and care coordination via telehealth platform
- Collaborate with interdisciplinary teams to develop personalized care plans for chronic disease management
- Provide clinical guidance and education to patients and caregivers through secure video consultations
- Utilize EHR systems to maintain accurate patient records and documentation
- Respond to urgent patient inquiries within established clinical protocols
- Participate in quality improvement initiatives and telehealth program development
- Maintain compliance with HIPAA and Georgia nursing board regulations
Qualifications
- Active Georgia RN license (Compact License preferred)
- 3+ years clinical experience in acute care, home health, or telemedicine
- Bachelor of Science in Nursing (BSN) required; MSN preferred
- Proficiency with telehealth platforms and EHR systems
- Strong critical thinking and decision-making abilities
- Excellent communication skills with diverse patient populations
- Certification in telehealth or telemedicine (preferred)
- Ability to work independently with minimal supervision
