Responsibilities
- Deliver virtual patient assessments via telehealth platform
- Coordinate care plans with multidisciplinary teams
- Document patient encounters in EMR systems
- Provide health education to patients remotely
- Monitor chronic conditions through virtual monitoring
- Collaborate with on-site staff for complex cases
- Participate in virtual case conferences
Qualifications
- Active RN license in California
- Recent nursing graduate or <1 year experience
- BLS certification (required, ACLS preferred)
- Proficient with telehealth technology
- Strong communication skills
- Ability to work independently
- San Diego residency required
