Responsibilities
- Conduct comprehensive virtual health assessments and develop patient-centered care plans
- Administer medications, monitor chronic conditions, and provide telehealth triage services
- Educate patients on disease management, medication adherence, and preventive care
- Collaborate with physicians and specialists to coordinate patient care across the continuum
- Maintain accurate electronic health records (EHR) and ensure HIPAA compliance
- Participate in quality improvement initiatives and clinical protocols
- Provide crisis intervention and emergency response support as needed
Qualifications
- Active Georgia RN license in good standing
- Minimum 2 years of clinical nursing experience
- Bachelor of Science in Nursing (BSN) preferred
- Strong telehealth or virtual care experience highly desirable
- Excellent communication and critical thinking skills
- Proficiency with EHR systems and telehealth technology
- Ability to work independently while collaborating effectively in virtual teams
- Certification in Telehealth Nursing (C-T) a plus
