Responsibilities
- Provide virtual patient assessments and triage services via telehealth platform
- Deliver patient education and medication management remotely
- Collaborate with interdisciplinary teams to develop care plans
- Document clinical encounters accurately in EHR systems
- Respond to after-hours patient inquiries during weekend shifts
- Monitor patient vitals and health data through remote monitoring tools
- Participate in continuous quality improvement initiatives
Qualifications
- Active WI RN license with unrestricted status
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care certification preferred
- Strong proficiency with electronic health records
- Excellent communication and critical thinking skills
- Ability to work independently during weekend shifts
- BLS certification required
- Experience with remote monitoring technologies
