Responsibilities
- Deliver comprehensive patient assessments and develop personalized care plans via telehealth platforms
- Administer medications and treatments with precision while monitoring patient responses
- Collaborate with multidisciplinary teams to ensure coordinated care delivery
- Maintain accurate electronic health records and documentation in compliance with HIPAA standards
- Provide patient education on chronic disease management and medication adherence
- Triage urgent patient inquiries and respond to emergency situations with clinical expertise
- Participate in quality improvement initiatives and continuous process enhancement
Qualifications
- Active RN license in the state of Texas with no disciplinary actions
- Bachelor of Science in Nursing (BSN) preferred; ADN with relevant experience considered
- Minimum 2 years of clinical nursing experience in acute care or telehealth
- Proficiency in EHR systems (e.g., Epic, Cerner) and telehealth technologies
- Strong critical thinking skills and ability to work independently in remote settings
- Certification in BLS and ACLS required; Telehealth certification preferred
- Exceptional communication skills with ability to build rapport virtually
