Responsibilities
- Deliver comprehensive virtual patient assessments and care coordination
- Manage chronic conditions and acute care triage via telehealth platform
- Collaborate with multidisciplinary teams for integrated care plans
- Maintain accurate EHR documentation while adhering to HIPAA compliance
- Provide patient education on health management and medication adherence
- Participate in quality improvement initiatives and clinical protocols
- Respond to after-hours patient inquiries within SLA requirements
Qualifications
- Active Texas RN license (compact license preferred)
- BSN with 2+ years clinical experience
- Telehealth or virtual care experience strongly preferred
- Proficiency with EHR systems and telemedicine technology
- Strong diagnostic and critical thinking skills
- Excellent written/verbal communication abilities
- Ability to work independently with minimal supervision
