Responsibilities
- Conduct comprehensive virtual patient assessments and triage via secure video conferencing tools.
- Monitor patient health status remotely and report critical changes to physicians immediately.
- Manage and update Electronic Health Records (EHR) with strict adherence to confidentiality protocols.
- Coordinate virtual follow-up appointments, medication reminders, and chronic disease management plans.

Qualifications
- Valid and active Certified Nursing Assistant (CNA) license in the State of New York.
- Proven experience in telehealth, remote patient care, or a similar virtual healthcare setting.
