Responsibilities
- Conduct comprehensive virtual patient assessments, triage, and consultations via secure video conferencing.
- Manage patient care plans, medication reconciliation, and follow-up instructions remotely.
- Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to ensure holistic care.
- Document all patient encounters accurately and promptly in electronic health records (EHR).
- Educate patients on disease prevention, self-care management, and the use of telehealth technology.
- Maintain strict compliance with HIPAA regulations and company privacy policies at all times.
Qualifications
- Active RN or LPN license in the state of Ohio.
- Minimum of 2 years of nursing experience, preferably in telehealth, primary care, or acute settings.
- Strong proficiency with Electronic Health Records (EHR) systems (Epic, Cerner, etc.).
- Excellent verbal and written communication skills with a patient-centric approach.
- Reliable high-speed internet and a dedicated, professional home workspace.
- Ability to remain calm and composed in high-pressure situations while managing multiple patients.
