Responsibilities
- Conduct comprehensive telehealth assessments for patients presenting with acute and chronic conditions
- Provide clinical guidance and triage recommendations via secure virtual platforms
- Collaborate with physicians and specialists to coordinate patient care pathways
- Maintain accurate electronic health records (EHR) documentation
- Manage high call volumes while maintaining empathy and professionalism
- Educate patients on self-care protocols and appropriate next steps
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Minnesota with 3+ years clinical experience
- BLS certification; ACLS preferred
- Proven triage experience in emergency, urgent care, or telehealth setting
- Exceptional communication and critical thinking skills
- Proficiency with EHR systems and telehealth technology
- Ability to work independently with minimal supervision
- Strong decision-making abilities in high-pressure situations
- Experience with remote work tools and virtual collaboration platforms
